Licensed Practical Nurse (LPN) - Memory Care

New Lenox, Illinois - Part-Time (NOC Shift)

Join our compassionate and caring team at Charter Senior Living's Cottages of New Lenox as a Memory Care LPN ! We're seeking a dedicated and experienced LPN to provide high-quality care to our residents living with memory loss in a supportive and inclusive environment.

Schedule:

• Every Friday: 7:00 PM – 7:00 AM

• Every other Sunday: 7:00 PM – 7:00 AM

Your Impact as a Memory Care LPN:

As an LPN, you will play a critical role in ensuring residents receive safe, compassionate, and high-quality care. Your responsibilities will include:

• Conducting resident assessments and supporting individualized care plans

• Administering medications and treatments accurately

• Monitoring resident conditions and communicating changes appropriately

• Ensuring proper documentation of care and services

• Supporting and guiding caregiving staff during overnight shifts

• Promoting a calm, safe, and supportive environment for memory care residents

• Helping residents maintain dignity, comfort, and quality of life

Core Responsibilities:

• Medication administration and treatment management

• Resident monitoring and clinical support during NOC hours

• Accurate and timely documentation

• Collaboration with care team and leadership

• Adherence to all state regulations and clinical standards

What We're Looking For:

• Active LPN license in Illinois

• Experience in memory care, assisted living, or long-term care preferred

• Strong clinical judgment and attention to detail

• Ability to work independently during overnight shifts

• Compassionate, patient, and team-oriented approach

• Reliable and committed to consistent scheduling
