Social Worker MSW

The Social Worker MSW is fully engaged in providing quality/no harm, customer service and stewardship by utilizing advanced psychosocial skills to facilitate the coordination of patient care through assessment, planning, intervention and evaluation. The Social Worker MSW uses current knowledge to effectively evaluate, promote and utilize hospital and community resources and services to achieve optimal outcomes, serving as a role model for coordination of psychosocial services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers.

Primary responsibilities include:

1. Assisting with coordination of referrals for post-acute services as appropriate in a timely manner.
2. Effectively communicating with the patient/caregiver and physician on payer or other financial issues.
3. Coordinating and documenting complex discharge plans.
4. Collaboratively determining desired discharge goals and communicating all discharge plans and options to patient/family, physicians and staff.
5. Participating in patient care conferences/ethics consults as indicated.
6. Providing counseling and researching placement alternatives for substance abuse, mental health, homeless and indigent patients.
7. Facilitating and participating in transfer of Baker Act and Marchman placements to mental health facilities. Providing therapeutic interventions with patients and families as needed.
8. Completing mandatory reporting processes.
9. Facilitating referrals for post-discharge psychosocial, therapeutic, and any other needs as assigned.
10. Identifying system/process breakdowns and improvement opportunities.
11. Communicating avoidable delays utilizing the escalation process to remove barriers to progression of care.
12. Ensuring compliance with regulatory standards, identifying risk management quality issues and reporting as required.

Minimum qualifications include:

• Education: Masters of Social Work or Bachelors of Social Work with Master's Degree in related field.
• Licensure: None required.
• Certification: Current American Heart Association Basic Life Support Healthcare Provider Completion Card upon hire and maintained.
• Work Experience: One (1) year of previous social work experience.
• Knowledge/Skills/Abilities: Basic computer skills including knowledge of MS Office Outlook and Word. Current knowledge of healthcare economic issues impacting acute care. Working knowledge of social work scope of practice and current issues impacting healthcare. Understanding of payer/reimbursement practices and regulations. Ability to understand and comply with Federal and state regulatory standards of care. Ability to effectively handle multiple demands in a short timeframe.

Preferred qualifications include:

• Certification: CCM or ACM Case Management Certification.
• Work Experience: Case Management experience in a hospital setting.

Physical requirements (Sedentary): Majority of time involves sitting or standing; occasional walking, bending, stooping. Long periods of computer time or at workstation. Light work that may include lifting or moving objects up to 20 pounds with or without assistance. May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise. Communicating with others to exchange information. Visual acuity and hand-eye coordination to perform tasks. Workspace may vary from open to confined. May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
