- Review patient records for diagnosis and medical history
- Establish goals and develop treatment plans for patient independence and function
- Re-assess treatment plans and progress towards goals as needed
- Communicate patient progress with treatment team and family
- Consult with medical providers regarding patient status and treatment
- Recommend adaptive equipment to enhance rehabilitation
- Maintain documentation in compliance with regulations
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development and obtain continuing education
- WV PT license (compact licenses accepted)
- Experience with HCHB preferred
- Experience in OASIS and Home Health
