Responsibilities

• Assess residents’ speech, language, voice, cognition, and swallowing needs and document findings.
• Develop and implement individualized treatment plans; monitor progress and adjust as needed.
• Provide direct therapy to residents and support caregivers with strategies to reinforce communication goals.
• Collaborate with an interdisciplinary team, including nursing, rehab therapists, and physicians, to coordinate care and support discharge planning.
• Maintain accurate and timely documentation in the patient record; ensure compliance with standards, privacy, and safety protocols.
• Communicate clearly with families and caregivers regarding goals, progress, and next steps.

Requirements

• Ability to work in a long-term care setting
• Proficiency in evaluating and treating adult and geriatric populations with communication and swallowing disorders
• Strong documentation and communication skills; experience with interdisciplinary teams
• Commitment to a 13-week travel assignment with guaranteed 35 hours per week
