Mercycare Community Physicians
Mercy Medical Center in Cedar Rapids, IA, seeks a Patient Care Coordinator to support care coordination and discharge planning for patients with complex needs. The role involves daily communication with physicians and staff, and collaboration across programs to ensure timely, quality care delivery.
Required is a bachelor’s degree in nursing with current Iowa RN licensure. Prior case management and utilization management experience are preferred, with attention to InterQual criteria and payer
