Pharmacist
The Pharmacist provides safe, accurate, and timely pharmacy services for inpatient and outpatient settings. Responsibilities include reviewing and verifying medication orders, dispensing and distributing medications, providing drug information and patient counseling, and supporting medication safety and quality initiatives in collaboration with providers, nursing, and other members of the care team.
Essential Duties and Responsibilities
- Reviews and verifies medication orders for appropriateness, accuracy, allergies, interactions, contraindications, duplicate therapy, and dose adjustments (e.g., renal/hepatic) in accordance with hospital policy.
- Dispenses, prepares, and distributes medications and supplies for inpatient and outpatient care, ensuring correct drug, dose, route, patient, labeling, and documentation.
- Uses automated dispensing cabinets and barcode medication verification processes where applicable to support safe medication distribution.
- Provides drug information and clinical support to providers, nursing, patients, and caregivers, including counseling as appropriate.
- Supports medication safety by identifying, documenting, and communicating medication-related problems, and by reporting errors, near misses, and adverse drug events per policy.
- Assists with medication reconciliation and transitions of care processes as assigned.
- Performs and/or verifies non-sterile and sterile compounding activities as assigned and as trained/competency-assessed, in accordance with department policy and required standards.
- Maintains accurate patient profiles and documentation in the pharmacy information system/EHR and ensures timely communication of clarifications to prescribers.
- Documents clinical interventions and communicates medication-related recommendations and order clarifications promptly to prescribers and the care team.
- Supervises and provides work direction to pharmacy technicians and other pharmacy staff; supports training, competency, and workflow priorities.
- Complies with HIPAA and all applicable federal/state regulations, accreditation standards, and hospital safety policies (including OSHA).
- Participates in continuous quality improvement activities (e.g., audits, policy/procedure updates, formulary/medication-use evaluations) as assigned.
- Provides coverage across inpatient and outpatient pharmacy services and other areas, based on department needs, maintaining continuity of care and consistent service standards.
- Maintains required licensure and completes annual education, training, and competency requirements.
Job Requirements
Education: Graduate of an accredited School/College of Pharmacy (ACPE-accredited). PharmD or BS in Pharmacy accepted.
Licensure/Certification: Licensed as a Pharmacist by the State of Indiana required.
Experience: Prior hospital pharmacy experience preferred but not mandatory; new graduates considered. Experience supporting both inpatient, outpatient workflows, clinical communication with prescribers/nursing, and medication order verification is a plus.
Knowledge/Skills/Abilities: Demonstrated clinical judgment and attention to detail; strong verbal and written communication skills; ability to prioritize work in a fast-paced environment; ability to collaborate effectively with interdisciplinary teams; working knowledge of pharmacy systems and electronic health records.
Supervisory Scope: Provides day-to-day work direction and oversight for pharmacy technicians and other pharmacy staff (training, task prioritization, and workflow support).
Schedule/Work Conditions: May include variable shifts, weekend/holiday coverage, and on-call responsibilities as scheduled. Work may include floating between inpatient, outpatient, retail pharmacy areas and entering patient care areas and pharmacy/compounding work areas as needed.
Physical Requirements: Frequent standing and walking; occasional sitting. Ability to frequently lift 25 lbs. Frequent reaching, stooping, and bending. Good visual acuity.
