Hospice RN Case Manager — Compassionate Patient Advocate

VIA Health Partners is seeking an experienced Registered Nurse (RN) in Lincolnton, NC to deliver and coordinate hospice and palliative care for patients and families. You will perform nursing assessments, develop and update care plans, and educate families to support independence and comfort. The role requires an active RN license, CPR certification, and at least 1 year of inpatient or hospice experience.
