This role is designed to deliver high‑value, patient‑centered primary care for adults aged 65 and older. Built on the Age‑Friendly Health System’s “5Ms” framework—What Matters, Medication, Mentation, Mobility, and Multi‑Complexity—this clinic will provide longitudinal primary and acute care services tailored to the needs of the aging population. At UCLA Health, you can: Provide comprehensive primary care for a smaller patient panel to enhance patient access and personalized care. Enjoy longer visit times with patients, allowing quality time. Work in a collaborative, interdisciplinary team including Clinical Advisor RNs, Comprehensive Care Coordinators, and Licensed Clinical Social Workers to support patient care. Dedicate 32 hours per week to direct patient care, with additional time allocated for administration and chronic care management. Develop professionally through specialized education focused on chronic care management, value‑based care, and population health strategies. Collaborate with the Senior Care Model Medical Director to drive quality improvement initiatives and evaluate new care delivery models.
