This full-time position is for a home care specialist who will work directly with patients in their homes, focusing on orthopedic rehabilitation. Work Environment

The Physical Therapist will work directly in patients' homes, ensuring a comfortable and personalized setting for therapy. This onsite approach allows for a tailored experience that addresses each patient's unique challenges and lifestyle. You will collaborate closely with a team of two other physical therapists and one physical therapy assistant (PTA), sharing insights and strategies to enhance patient care. This role allows for flexibility in scheduling while maintaining a commitment to providing high-quality treatment.

Key Responsibilities

• Conduct thorough assessments to identify each patient's goals and requirements.
• Manage a patient caseload efficiently, typically seeing one individual per hour, ensuring personalized attention and care.
• Develop comprehensive treatment plans that may include manual therapy, exercise education, and progressive rehabilitation techniques.
• Implement cost-effective treatment methods aimed at improving patients' range of motion and mobility.
• Promote alternatives to medication and surgical procedures by focusing on evidence-based physical therapy interventions.
• Motivate patients throughout their rehabilitation journey, adjusting treatment plans as necessary based on regular evaluations.
• Document all patient interactions and treatment outcomes meticulously for continuity of care and compliance.
• Collaborate with other healthcare professionals, such as physicians and occupational therapists, to ensure integrated patient care.
• Encourage healthy lifestyle choices by educating patients on strength-building exercises, flexibility routines, and balance training.
• Maintain a positive patient experience by fostering an encouraging and supportive atmosphere during sessions.

Patient Population / Client Focus

The patients you will work with primarily include individuals focusing on orthopedic rehabilitation after surgery or injury, as well as those with chronic pain conditions. Your role will be to provide a higher quality of life through improved mobility, support them in their recovery process, and help them achieve their therapeutic goals. Your compassion and expertise will make a difference in their rehabilitation journey as they work towards independence and improved physical capabilities.

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Master's or Doctorate degree (DPT) is preferred.
• Valid state license to practice as a Physical Therapist.
• Strong professional record with no disciplinary actions taken against your license to ensure accountability and trust.
• Proficient in manual therapy techniques and exercise protocol development.
• Familiarity with electronic medical records (EMR) systems, particularly WebPT, is a plus.

Preferred Experience

• Previous experience in an orthopedic outpatient setting is strongly preferred to effectively address patient needs.
• Expertise in developing individualized treatment plans and protocols for diverse patient populations.
• Demonstrated ability to work collaboratively within a healthcare team to optimize patient outcomes.
• Experience in patient education and promoting lifestyle changes beneficial for long-term health.

Compensation and Benefits

We offer a competitive compensation package with a salary range of $90,000 to $120,000 annually, commensurate with experience and qualifications.
