About the Position

Job Title: Visit Nurse Profession: Registered Nurse Specialty: Home Health Duration: 3 Month Temporary Assignment Shift: Day Shift Hours per Shift: 8 Experience: Minimum of 2 years of recent experience in home health, acute, or long-term care License: Current Registered Nurse license Certifications: Current BLS certification Must-Have: Valid driver's license; proof of insurance on adequate automobile; reliable automobile transportation to travel to patient residences; driving record in compliance with standards; cell phone with activated voicemail; computer internet connectivity Description: The Visit Nurse helps provide continuity of care by performing nursing visits for patients as delegated by the Case Manager RN. The Case Manager RN is responsible for the overall planning and coordination of care for a caseload of patients. The Visit RN is a member of the interdisciplinary team providing nursing care in patients' residences as prescribed by the physician. Care must be compliant with applicable laws, regulations, and policies of the agency. The Visit RN will keep the Case Manager RN informed about patient progress, recommended actions, physician orders, and plans for upcoming visits. Individuals in this role are expected to apply their knowledge of nursing principles and practices. Demonstrating problem-solving skills and the ability to make independent decisions in complex work situations is essential.
