This includes coordinating assessment schedules, reviewing supporting clinical documentation, monitoring Medicare requirements, participating in care planning, and helping ensure services are accurately reflected for clinical and reimbursement purposes.The MDS Coordinator serves as an important resource for the interdisciplinary team and helps maintain compliance with applicable federal, Missouri, Medicare, Medicaid, and facility requirements.ResponsibilitiesCoordinate the Resident Assessment Instrument and MDS processes for facility residentsDevelop and maintain an interdisciplinary schedule for MDS assessments and care plan reviewsEnsure MDS assessments are completed accurately and within required timeframesEstablish and communicate Assessment Reference Dates in collaboration with the interdisciplinary teamCoordinate completion of required assessment sections by appropriate members of the interdisciplinary teamReview the medical record to ensure documentation supports MDS coding, resident conditions, treatments, services, and functional abilitiesMonitor documentation for accuracy, completeness, consistency, and regulatory complianceWork with nursing, therapy, dietary, social services, activities, and other departments to obtain necessary assessment informationParticipate in daily clinical meetings, stand-up meetings, Medicare meetings, and other resident review processes as assignedHelp identify significant changes in resident condition and determine whether an assessment or care plan update is requiredCoordinate with the Director of Nursing or designee regarding physician orders, clinical reports, and resident changesEnsure the care plan reflects identified resident needs, preferences, risks, strengths, and measurable goalsFacilitate interdisciplinary care plan meetings and encourage resident and responsible-party participationMonitor Medicare eligibility, skilled coverage, certification requirements, and supporting documentationHelp ensure continued-stay documentation is completed accurately and within required timeframesSupport accurate clinical reimbursement and Patient-Driven Payment Model processesReview assessment information for potential coding discrepancies before submissionTrack MDS submissions, corrections, modifications, accepted assessments, and completion deadlinesComplete regular audits of MDS coding, care plans, and supporting clinical documentationReport identified trends, risks, and audit findings to nursing leadership and the Quality Assurance and Performance Improvement CommitteeProvide MDS, documentation, and care-planning education to clinical personnel as neededStay informed about changes to MDS, RAI, Medicare, Medicaid, and clinical reimbursement requirementsAssist with documentation preparation for regulatory surveys, audits, and medical record reviewsMaintain resident privacy, confidentiality, and protected health informationPerform other duties related to the position as assignedQualificationsGraduate of an approved practical or registered nursing programCurrent, active, and unencumbered RN or LPN license in the State of MissouriCurrent BLS/CPR certificationAt least 18 months of recent experience as an MDS Coordinator requiredExperience with clinical reimbursement and Medicare documentation requiredWorking knowledge of the MDS, RAI process, care planning, and skilled nursing documentationKnowledge of Patient-Driven Payment Model processes preferredResident Assessment Coordinator certification, such as RAC-CT, preferredSkilled nursing or long-term care experience requiredStrong clinical assessment, critical-thinking, communication, and organizational skillsStrong attention to detail and commitment to accurate documentationAbility to manage multiple assessments, deadlines, and competing prioritiesAbility to communicate expectations and provide education to interdisciplinary team membersProficiency with electronic health records and MDS-related softwareDependable attendance and reliable transportationProfessional and collaborative demeanorCommitment to treating every resident with compassion, dignity, and respectAbout Avalon ViewAvalon View Health and Wellness is a skilled nursing, rehabilitation, long-term care, and memory care community located in Liberty, Missouri.
