Physical Therapist – FloatSenior Care Partners PACE is seeking a Physical Therapist – Float to provide compassionate, participant-centered care across our Kalamazoo and Calhoun County locations. This is an opportunity to use your clinical expertise in a dynamic setting where you'll help older adults maintain mobility, independence, safety, and quality of life—both at our PACE centers and in the community.What You'll DoEvaluate participants' mobility, physical abilities, and rehabilitation needs.Develop and implement individualized physical therapy plans of care.Provide therapy through therapeutic exercise, modalities, manual therapy, and other evidence-based interventions.Float between Kalamazoo and Calhoun counties based on participant and program needs.Collaborate closely with the Interdisciplinary Team (IDT) to coordinate participant-centered care.Assess and recommend adaptive and durable medical equipment and support equipment transitions.Educate participants, families, and caregivers to promote safety and independence.Support physical wellness programming and help develop innovative programs for participants.Provide guidance and mentorship to PTAs, rehab tech PCAs, and PT students when applicable.Support participants transitioning home from hospitals, skilled nursing, assisted living, and other care settings.Participate in quality improvement initiatives and promote exceptional customer service.What You BringDoctorate in Physical Therapy (DPT), unless grandfathered under applicable requirements.Graduate of an approved physical therapy program.Current, unencumbered Michigan Physical Therapist license.At least 1 year of experience working with frail, elderly, or long-term care populations.Strong communication, organization, problem-solving, and collaboration skills.Working knowledge of Microsoft Office.Current AHA CPR certification or willingness to obtain certification.Commitment to our core values of Belonging, Partnership, Ownership, and Excellence.Why This Role MattersAt Senior Care Partners PACE, you'll be part of a collaborative team dedicated to helping older adults live safely and independently in the community.
