Position Overview: We are seeking a compassionate and detail-oriented Registered Nurse Assessment Coordinator to join our team. The RNAC will play a critical role in assessing and documenting patient needs, coordinating care plans, and ensuring regulatory compliance. This position offers the opportunity to work in a supportive environment where your expertise will contribute to improving patient outcomes. Key Responsibilities: Conduct comprehensive patient assessments to determine care needs. Collaborate with healthcare professionals to develop and update individualized care plans. Ensure compliance with Medicare and Medicaid regulations and guidelines. Accurately complete and submit MDS (Minimum Data Set) assessments. Monitor patient progress and adjust care plans as necessary. Provide education and support to clinical staff regarding assessment and documentation best practices. Participate in interdisciplinary team meetings to discuss patient care and coordination. Maintain accurate and up-to-date patient records. Qualifications: Current Registered Nurse (RN) license in Pennsylvania. Minimum of one year of experience in long-term care or similar healthcare settings. Strong knowledge of MDS assessments and Medicare/Medicaid regulations. Excellent communication and interpersonal skills. Detail-oriented with a commitment to accuracy and compliance. Ability to work collaboratively in a team-oriented environment. Proficient computer skills and familiarity with electronic health records (EHR) systems.
