Description

• Conduct assessments of patients in both inpatient and outpatient environments, designing personalized treatment plans tailored to their needs
• Implement physical therapy interventions emphasizing mobility, strength, balance, and functional enhancements
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize therapeutic outcomes
• Educate patients and their families regarding therapeutic objectives, discharge plans, and home exercise regimens
• Maintain comprehensive documentation of evaluations, treatments, and patient progress per facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure seamless, integrated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• Preferred CPR certification
• Specialty certifications (i.e., neurological, geriatric) are advantageous, though not mandatory

Skills

• Previous experience is strongly preferred
