Pay: $60,000 - $75,000

About Youth Opportunity & Crestview Youth Academy

Youth Opportunity is a nationally recognized leader in residential and behavioral health treatment for youth. Crestview Youth Academy in Crestview, Florida, includes a secure residential program serving 56 males ages 13 to 21 who receive mental health and substance use treatment services. The facility also includes a co-located, non-secure residential program serving 24 males ages 13 to 18 who meet the criteria for Borderline Developmental Disability or Developmental Disability services.

Services are provided 24 hours a day, seven days a week, using an individualized, trauma-informed approach. Youth participate in individual, family, and group counseling; social and life skills training; victim-impact awareness; restorative justice programming; and specialized mental health, substance use, and developmental disability services.

The program’s goal is to help youth complete their individualized treatment plans, build the skills needed for long-term success, and prepare for successful reintegration into their homes and communities.

Position Summary – Director of Case Management Job in Crestview, FL

We are hiring a Director of Case Management to lead and support the case management team at Crestview Youth Academy. This position provides supervision, guidance, and mentoring to case managers while also maintaining an assigned caseload.

The Director of Case Management coordinates admissions, discharges, transportation, family visits, court communication, and collaboration with referring agencies. This position also ensures that case records, reports, correspondence, and other required documentation meet court, agency, contractual, and Youth Opportunity requirements.

This role is ideal for an experienced human services professional with strong leadership, communication, case coordination, and records-management skills.

Director of Case Management Responsibilities

  • Supervise, guide, mentor, and support all case management staff
  • Assign and monitor caseloads while maintaining an assigned caseload as needed
  • Assess newly admitted youth by reviewing records, communicating with referring agencies, and interviewing youth and their families
  • Coordinate admissions, discharges, transportation, and transition planning with case managers and other facility departments
  • Coordinate case management activities and communication with courts, referring agencies, families, and other outside partners
  • Coordinate family visits and other family-engagement activities with the case management team
  • Serve as a resource for staff working with courts, referring agencies, families, and outside systems
  • Ensure required reports, correspondence, case notes, and other documentation are accurate, timely, and compliant with court, agency, contractual, and Youth Opportunity guidelines
  • Review case files and records to ensure documentation is complete, current, and properly maintained
  • Assist the Facility Administrator with youth- and family-related concerns
  • Collaborate with facility leadership and members of the multidisciplinary treatment team
  • Conduct periodic case-record reviews and support performance-improvement activities
  • Communicate youth progress, needs, and transition recommendations to the appropriate individuals and agencies

Other Duties Include

  • Maintain a high standard of professional and role-model behavior for youth
  • Participate in required training and professional development activities
  • Report any acts, incidents, or conditions reflecting the possibility of inappropriate youth-to-youth or staff-to-youth relationships
  • Report any use of physical force and all unusual incidents according to established guidelines
  • Perform other duties as assigned or needed

Required Qualifications

  • Associate degree from an accredited college or university
  • Two or more years of experience working with children or adolescents with serious emotional, behavioral, or substance use needs
  • Candidates with a master’s degree in an appropriate field may be considered without two years of professional experience
  • Advanced verbal and written communication skills
  • Strong leadership, organizational, documentation, and records-management skills
  • Ability to communicate effectively with youth, families, courts, referring agencies, and facility staff
  • Ability to manage multiple responsibilities and meet required deadlines
  • Ability to pass required background checks and pre-employment screenings

Preferred Experience

  • Bachelor’s degree in counseling, social work, psychology, rehabilitation, special education, or a related human services field
  • Previous supervisory or case management leadership experience
  • Experience working with local or state agencies, courts, residential programs, behavioral health programs, or juvenile services
  • Experience with group leadership, group dynamics, and experiential education
  • Experience supervising documentation, admissions, discharge planning, and family engagement activities

Other Requirements

Physical Demands: Must be in good physical health and emotionally stable. Must be able to move throughout the property, participate in youth activities, and transport youth or staff when required. Must meet applicable communicable-disease and health requirements.

Other: Must maintain an appropriate and valid state driver’s license. Must meet all applicable state criminal background check and pre-employment screening requirements.
