Must have BLS certification (American Heart Association) NRP preferred but not required License/Certificate verification must be dated within 30 days of start Must complete Trinity Specialty Specific Competency Testing within a year Must complete Trinity Skills Checklist for clinical contractors Must provide proof of COVID-19 vaccine or approved declination/exemption Must provide proof of MMR vaccination or IgG titer Must have started Hepatitis B vaccination series prior to start Must provide proof of Influenza vaccine or approved declination/exemption Must provide proof of 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest xray if positive TB Must provide proof of Varicella vaccination or IgG titer Must provide Physical/Health Statement from exam within last year Must complete Trinity 10 Panel Drug Screen within 30 days of start Must complete Trinity Code of Conduct Form Must complete Trinity I9 form Must complete Trinity Confidentiality of Patient & Proprietary Information and Information System Security Agreement Must complete Trinity Integrity and Audit Services Form Must complete Trinity Respirator Evaluation with physician signature Must complete Trinity Profile including resume, skills checklist, and 2 professional references Must complete Trinity Conflict Of Interest Disclosure Form Must complete Trinity Criminal Background Check within 30 days of start Must complete Trinity Education Verification Must complete Trinity EPLS report within 30 days Must complete Trinity GSA/OIG report within 30 days Must complete Trinity Joint Commission Testing (CORE Exam) within a year Must complete Trinity NSO report within 30 days Must complete Trinity System Access Form one week before start Must provide current, non-expired Tdap vaccine or approved declination if no pediatric interaction Must complete Trinity OFAC within 30 days Schedule Information
