Providing essential patient navigation services, the full-time Care Navigator for Immigrants will assist with care coordination, referral management, and access to health resources in a fully remote environment. Key responsibilities Manage transitions of care for patients across various healthcare settings to ensure continuity and compliance Address barriers to care and facilitate connections to appropriate services, enhancing patient experience and retention Track and document referrals and patient interactions to promote safety and team awareness Required qualifications High School Diploma or GED required 1-3 years of experience in a clinical care setting required Intermediate knowledge of community resources for reentry within the Denver Health integrated care system Bilingual in English/Spanish preferred Knowledge of electronic health records systems, such as Epic
