Job Description: Physical Therapist (PT)
Transformative Pain Care & Physical Therapy is seeking a motivated and compassionate Physical Therapist to join our growing practice in Delta, Colorado. We prioritize patient-centered care, professional growth, and a healthy work-life balance.
As a PT on our team, you will focus on evidence-based treatments to address pain and improve functionality, serving as a trusted partner in our community’s wellness journey.
Compensation & Benefits
Hourly Rate: $38–$60/hour (Based on experience and specialty)
Sign-On Bonus: $5,000 total
$2,000 upon acceptance
$1,000 at the completion of years 1, 2, and 3
Health Insurance: Basic coverage provided
Continuing Education: Annual allowance for professional development
Time Off: Paid Time Off (PTO) and major holidays provided
Key Responsibilities
Evaluate patients and develop individualized treatment plans.
Provide hands-on therapy and therapeutic exercise programs to restore mobility.
Educate patients and caregivers on injury prevention and wellness strategies.
Collaborate with a multidisciplinary team to optimize patient outcomes.
Maintain accurate, timely clinical documentation.
Qualifications
Active Physical Therapist license in the State of Colorado.
Graduate of an accredited Physical Therapy program.
CPR and BLS Certified.
Strong interpersonal skills and a commitment to patient-centered care.
Preferred: Specialty certifications (OCS, SCS, TPS) or an interest in Pain Management.
