PRN Home Health Physical Therapist (PT)

Are you a compassionate Physical Therapist looking for the flexibility of PRN while providing exceptional one-on-one patient care? What You'll DoAs a Home Health Physical Therapist, you will:
- Perform comprehensive patient evaluations and establish individualized treatment plans.
- Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence.
- Evaluate patient progress and modify treatment plans to achieve optimal outcomes.
- Educate patients and caregivers on exercises, safety, and disease management.
- Maintain accurate, timely documentation in accordance with company standards.
- Collaborate with physicians, nurses, and the interdisciplinary care team.
- Follow infection control and universal precaution guidelines.
- Deliver compassionate, high-quality care while promoting Choice Health At Home's mission and values.
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist (PT) license.
- Minimum of one (1) year of home health experience preferred.
- Current CPR certification.
- Valid driver's license and reliable transportation with current auto insurance.
- HomeCare HomeBase experience preferred.
- Ability to stand, walk, bend, kneel, squat, and sit for extended periods.
- Ability to safely lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
