Registered Nurse
Caretenders Home Health, part of LHC Group, is seeking a Patient Care Manager RN Hybrid to supervise clinical services and deliver nursing care to patients in their homes as prescribed by physicians. You will coordinate an interdisciplinary team, perform comprehensive assessments, manage referrals, and ensure continuity of high-quality care in accordance with state and federal regulations.
