Forest Manor Health & Rehab Center is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our skilled rehabilitation team. We currently have openings for both Full-Time and Per Diem (PRN) positions.

As a Speech-Language Pathologist, you will play a vital role in helping residents improve communication, cognitive-linguistic skills, and swallowing function, enabling them to achieve their highest level of independence and quality of life.

Responsibilities

Evaluate residents and develop individualized speech-language pathology treatment plans.
Assess and treat communication disorders, cognitive-linguistic impairments, speech and language deficits, and swallowing disorders (dysphagia).
Implement evidence-based therapeutic interventions to maximize resident outcomes and functional independence.
Collaborate with physicians, therapists, nursing staff, dietary services, and families to support resident goals and care plans.
Educate residents, families, and caregivers regarding communication strategies, swallowing precautions, and home programs.
Maintain accurate and timely documentation in accordance with facility policies, state regulations, and Medicare guidelines.
Participate in care planning and interdisciplinary team meetings.

Qualifications

Current Speech-Language Pathologist license in the State of Alabama.
Certificate of Clinical Competence (CCC-SLP) preferred.
Strong clinical, interpersonal, and communication skills.
Ability to work independently and as part of a multidisciplinary team.
Experience in skilled nursing, rehabilitation, or long-term care settings preferred but not required.
