Job Title: Clinical Pharmacist
Contract Details: Full-time

The Clinical Pharmacist will be responsible for providing pharmaceutical care to patients in a clinical setting. This includes dispensing medications, monitoring patient drug therapies, and collaborating with other healthcare professionals to ensure the best possible patient outcomes.
Qualifications:
- Minimum of 2 years of experience in Clinical Pharmacy
- Valid Pharmacist license in the state of New York (NY)
- Strong knowledge of pharmaceuticals, drug therapy, and medication management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Detail-oriented with strong organizational and time-management skills
- Proficient in computer systems and electronic medical records
- Ability to adapt to changing environments and handle multiple tasks simultaneously
Responsibilities:
- Dispense medications accurately and efficiently
- Monitor patient drug therapies and provide appropriate interventions
- Collaborate with healthcare professionals to develop and implement treatment plans
- Conduct medication reconciliation and review for potential drug interactions
- Educate patients on proper medication use and potential side effects
- Document patient care activities and maintain accurate records
- Stay updated on new medications and treatment protocols
- Participate in quality improvement initiatives and adhere to regulatory standards
- Assist with inventory management and medication ordering
