This will entail the administrative oversight of medical conditions for our special needs population.

Essential duties will be as follow:

• Provide medical oversight and guidance for special needs population
• Work with doctors’ offices for appointments / medication changes / medication refills.
• Track medication changes monthly and fax to pharmacy prior to monthly refills.
• Check in monthly medication and verify accuracy against the MAR and consultation sheet.
• Track individuals annual Physicals, TB test, Vision, Dental, Mammogram and OBGYN
• Update Health Risk Screening Tool
• Update medication reduction form for psychotropic medication changes
• Emergency phone after hours on-call rotation
• Perform employee medication training
• Write health care protocols as needed

The Chanan Foundation is seeking an individual that is a current State Licensed Registered Nurse. The Chanan Foundation is a team oriented company that is eager to help in assisting the successful candidate into their role with the Chanan Foundation. All members of the management team take turns being on call for emergencies after hours and on weekends.

Job Type: Full-time

Salary: $55,000.00 to $60,000.00
