Job Title: Mobile Wound Care Nurse Practitioner (AGNP/CNP) | Senior Living Rounding .

Company: Twin Cities Physicians.

Compensation: $120,000 to $160,000 (Base + RVUs earning potential)

Benefits: Full Benefits Package (FT status only) .

Location: Greater Twin Cities Metro Area, MN.

Schedule: Full-Time or Part-Time | Flexible Rounding Model .

Position Summary:

As a Wound Care focused Nurse Practitioner , you will provide hands-on care inside senior communities, managing complex wounds while building ongoing relationships with your patients. This role is ideal for someone who values independence, variety, and the ability to directly influence patient outcomes . You will serve as a key clinical partner to staff, families, and physicians within your assigned communities.

Responsibilities:

• Provide on-site rounding and advanced wound care services .

• Develop and manage individualized treatment plans .

• Collaborate with physicians and interdisciplinary teams .

• Communicate with families and community staff .

• Manage medications, labs, and documentation.

Qualifications:

• Active Certified Nurse Practitioner (CNP) license in Minnesota .

• Experience in geriatrics, primary care, or wound care preferred .

• Valid driver’s license and reliable transportation.

• Passion for serving the senior population .

Military Occupational Classification (MOC) Codes:

• Army: Nurse Practitioner (66P).

• Navy: Nurse Practitioner (1972).

• Air Force: Nurse Practitioner (46Y3H).

• Marines: Nurse Practitioner (NP).

• Coast Guard: Health Services Technician (HS).
