We are currently seeking a full-time Registered Nurse Case Manager who is passionate about providing compassionate, individualized care to patients and families during their most vulnerable moments. What You’ll Do: Manage a caseload of hospice patients, providing skilled nursing visits and coordinating all aspects of care. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Educate patients and families on hospice philosophy, care expectations, and symptom management. Act as the primary clinical contact for assigned patients, ensuring comfort, dignity, and quality of life. Maintain accurate and timely documentation that meets all regulatory and organizational standards. Participate in IDG meetings and collaborate with aides, chaplains, social workers, and medical directors. What We’re Looking For: Current Texas RN license in good standing. Minimum 1 year of nursing experience (Hospice, Home Health, or ICU/ER preferred). Strong clinical judgment and communication skills. Empathy, professionalism, and a heart for hospice care.
