Coordinate patient care as a team leader practicing Patient-Centered Medical Home principles Minimum Qualifications Education: Certification or graduation from a four (4) year college or university Licensure & Certifications: Current KY RN license Job Duties: 65% - Monitor patient vitals; detect changes in symptoms, health, and/or pain.
