Administer medications and treatments as directed by physicians and nursing staff. Monitor and document patient conditions and report any changes or concerns. Assist with daily living activities and support residents’ needs. Collaborate with the healthcare team to ensure comprehensive patient care. Maintain accurate records and adhere to all regulatory and facility standards. Licensed Practical Nurse (LPN) Requirements: Valid Licensed Practical Nurse (LPN) license in New Hampshire. Strong clinical skills and a compassionate approach to patient care. Excellent communication and interpersonal skills.Ability to work effectively both independently and as part of a team.
