Northgate Health Care Facility is looking to add a Licensed Practical Nurse (LPN) to its nursing team. Responsibilities

• Provide nursing care in accordance with resident assessments, care plans, and facility policies.
• Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice.
• Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties.
• Accurately document care, treatments, observations, and medication administration.
• Supervise and support Nursing Assistants to ensure quality resident care.
• Maintain up‑to‑date resident care information and communicate pertinent information during shift changes.
• Follow infection control, safety, resident rights, and confidentiality standards.
• Participate in interdisciplinary care planning and quality improvement initiatives.
• Assist with admissions, transfers, and other nursing responsibilities as assigned.

Requirements

• High School Diploma or equivalent required; Associate's Degree preferred.
• Valid NYS Licensed Practical Nurse (LPN) License required.
• Current CPR Certification required.Experience in a skilled nursing facility or with the geriatric population preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of medication administration, infection control, and resident care practices.
• Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team.
• Compassionate, resident‑centered approach to care.
