Provide medication management through an evaluation of medications for patients to ensure safe, effective utilization thatis tailored to specific medical conditions. Work with patients, providers, nurses, and other health care professionals toprovide access to specialty medicationsand ensure optimal patient outcomes. We approach patient care as a team andsupport each other as needed or directed to ensure the necessary patient care responsibilities are completed each day.This role is an exempt position with the potential for a hybrid on-site/work from home work model.Responsibilities:Pharmacy Practice Involvement:· Proactively maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in services, seminars, and conferences.· Actively participate in clinical meetings· Participate in training staff as directed· Cross train for other roles to provide periodic support as directed based on business needs· Participates in Clinical Subcommittees at MGBSP· Precepts students/residents as directedClinical Practice Expectations:Ensure all drugs are appropriately indicated, effective, safe and convenient for the patientIdentify, resolve and prevent drug therapy problemsDocument safety events at time of discovery through MGBSP safety event tool and assist in resolution under guidance of the Safety OfficerProvide immediate and scheduled patient counseling and respond to inquiries from patients, family members, and health care professionals concerning drug therapy in a timely mannerDocument therapeutic interventions based on patient and medication therapy evaluationFacilitate medication access by assisting with prior authorizations, drug therapy appeals, financial assistance, and other Patient Care Coordinator activities as directed.Provide On-call support on a scheduled rotationPerform patient evaluation as it pertains to medication management:Obtain patient histories (medication, allergy, disease) and apply these along with other patient data in the pharmaceutical care evaluationEvaluate and document patient outcomes at each encounterProvide patient education on medications and disease states in individual or group settingsCollect analyze, interpret and evaluate appropriate data relative to medication therapyEnsure that the patient understands the specific goals of therapy and that the patient and providers are working to achieve these goalsIdentify, resolve, and document patient specific drug therapy problems and issues in the patient’s best interestEvaluate the significance of signs and symptoms relative to disease processes and make recommendations for the modification of drug therapyEvaluate the significance and clinical importance of adverse reactions and complications associated with drug therapyDemonstrates Commitment to Fellow Employees:Seeks to understand and value differences between employeesTreats other employees with dignity and respectDemonstrates open, honest communication and behaviorResolves conflict in a positive mannerContributes to the productivity of the workgroupDemonstrates Commitment to Health System:Lives the values of dignity, integrity, service and compassionSupports the mission/visionSpeaks positively about Health SystemChooses to make Health System a better placeUse/s the Mass General Brigham values to govern decisions, actions and behaviors.
