Wellness Case Manager & Intake Coordinator
Endeavors Inc.
Endeavors is seeking a Case Manager/Intake Coordinator to assess eligibility and enroll CBP employees and eligible family members into the CBP Workforce Wellness Program in Detroit, MI. This full-time role requires up to 50% travel and a Bachelor's degree in a related field. You will conduct intake interviews, perform eligibility screenings, coordinate care, and develop personalized wellness plans.
