Clinic Care Coordinator – Baptist Health Memory Clinic NLR Job ID: 59172 Department: Baptist Health Memory Clinic NLR Shift: Day Working Hours: Monday - Friday 8am - 5pm Full Time Job Summary The Clinic Care Coordinator supports patients with chronic conditions and complex needs by providing care coordination, patient education, and resource navigation under the direction of the provider. This role identifies patients who would benefit from care management, monitors adherence to care plans, assists with overcoming barriers to care, and connects patients to internal and community resources. The Clinic Care Coordinator plays a key role in improving patient outcomes, continuity of care, and overall patient experience. Qualifications Bachelor's degree in nursing, social work, or other related clinical field (preferred) 3 to 4 years of experience (preferred) Ambulatory and/or care management experience (preferred) Current driver's license in good standing Reliable and insured transportation Other Information This job will be authorized 80.00 hours bi-weekly. #J-18808-Ljbffr
