Description
Capital Manor is seeking a qualified LPN to fill our Health & Wellness Navigator position. This position builds trusted relationships with residents and families, serves as a reliable resource, helps resolve concerns, and supports residents in achieving their highest possible level of health and well-being. In this role, the Navigator also helps residents access services and resources both within the community and externally, reducing barriers to successful health and wellness while supporting an independent living lifestyle.
The Health & Wellness Navigator assists residents in facilitating their health and wellness goals within the framework of the 8-Dimensions of Health & Wellness model including, but not limited to intellectual, occupational, social, environmental, emotional, physical, and health services, personal choice, and provide as much support as residents need to maintain overall well-being throughout their journey within the community.
The Health & Wellness Navigator, when necessary, will assist and guide residents, family members, and caregivers with resident transitions throughout the continuum of care to achieve the optimal level of well-being and appropriate level of care and communicating with all key resources and stakeholders.
PRINCIPLE DUTIES:
Essential Job Duties
- Meets the standards of the Health and Wellness Navigation Program and strives to achieve bronze, silver, and gold certification.
- Oversees cross-functional departmental support of all post-acute services within the community.
- Assists residents to transition within the continuum of care services to the appropriate levels of care offered within the community (IL, AL, MC). Assistance provided by introducing community resources and supportive services, such as Home Care, needed to obtain optimal levels of health.
- Leads the Radar Meeting. Collaborates with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy.
- Strives to achieve elevated levels of resident satisfaction.
- Coordinates/attends relative community meetings related to resident transitions, status updates, changes in resident condition, support services required, and appropriate level of care including but not limited to:
- Daily AL/MC stand up meeting
- Monthly Resident Advisory Council (RAC)
- Various support groups
- Maintains awareness and promotes all internal services to encourage resident health and wellness goals.
- Encourages resident participation in the community programs.
- Encourages resident participation and engagement in social events.
- Assists residents, family and community to prepare for and provide support during emergency situations.
- Maintains emergency medical information for each IL resident, updating them annually and assures confidentiality of all residents' information.
- Coordinates new resident orientation in collaboration with the Marketing and Wellness departments.
- Reviews emergency call notes/incident reports/concern forms from the community staff and others and will implement strategies to reduce risks specific to the resident as required.
- Acts as a resource for options and choices related to supportive strategies to improve health, function, and lifestyle. Ongoing coordination of services and resources of physical, emotional, and/or social needs of residents within the community.
- Serves as resident liaison and advocate, including coordinating assessments, offering consultations, and aiding with coordination of both internal and external resources. Assist with clinical care coordination, including blood pressure checks and vitals, offering wellness consultations, and guiding residents and families in coordination of health services (i.e., physician appointments, lab tests, x-rays, etc.).
- Initiates/interacts with resident and family members for issue resolution, risk reduction strategies, and opportunities to enhance the general well-being of the resident when change in condition necessitates additional services or a physical move across the continuum of care. Assist with residents and family members by offering support in dealing emotionally and psychosocially with the aging process and illness.
- Coordinates communication with physicians, families, and appropriate staff regarding resident's status. Works to prevent emergencies whenever possible.
- Oversees admission/discharge to/from other healthcare provider agencies (i.e., hospital, LTAC, Psychiatric hospital, inpatient hospice, etc.) by assisting residents to establish a plan of care to assist with transition back to the community supporting anticipated needs.
- Coordinates completion and review of results of the Independent Living Health & Well-being Questionnaire upon move in and coordinate additional interventions as required to ensure appropriate level of care, and services and resources are in place to optimize residents' wellness journey.
- Coordinates services with other departments, internal resources and/or external resources and involves other departments in programming as applicable.
- Collaborates with Manor Care and Memory Care staff to coordinate residents' short–term and long-term health care needs including admissions, discharges, and supportive services offered within the continuum of care.
Prepares and maintains required records, reports, studies, and surveys appropriate to navigate and communicate resident's condition and/or health services. Assures confidentiality of all residents' information contained therein.Develops and maintains listing of internal and external available health related resources. Evaluates these resources to ensure alignment with community/resident needs. Maintains viable relationships with all resources.Develops and maintains listing of internal and external personal service providers entering community. Evaluates these resources and ensures tracking of required documentation.
Partners with Wellness and Community Life Services/Life Enrichment to plan, implement, and evaluate health and wellness programming and health education opportunities which include topics of prevention, healthy lifestyles, and successful aging. Programming may include activities, clinics, events and support groups for residents, family, and/or staff.
Networks, plans, coordinates and contracts with qualified educators, instructors, and health professionals to provide program components. Coordinates services with other departments and involve other departments in programming as applicable.Remains available and actively engages in support of global community needs. Reviews daily check in log for Independent Living residents and coordinates room checks as needed to ensure safety and wellbeing of IL residents if applicable.Collaborates with other Manor Care leaders in the development of objectives, standards of care, and operating policies, procedures affecting the resident experience specifically related to health and wellness within the scope of responsibilities.Regular, reliable and punctual attendance is an essential job function of this position.Other duties as assigned
Qualifications
QUALIFICATIONS:Three to five years of experience working in the Senior Living Space, Independent Living, Assisted Living, and Memory Care or other health care setting with strong organizational skills, management experience with care coordination and care management with an interdisciplinary approach to care preferred.Graduate of an accredited college with an LPN, or RN. Bachelor's degree in social services, geriatrics, nursing, and or related senior living services preferred.Continuing Education:Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programsStrong interpersonal skills include the ability to motivate and encourage residents to achieve maximum independence and quality of life.Ability to problem-solve, make ethical recommendations, define appropriate boundaries and be resolution oriented.Excellent verbal and written communication skills for groups and individuals of different populations including persons with disabilities and Dementia.Effective organizational skills including delegation, managing multiple priorities, time management.Ability to collaborate with community management, internal and external resources/partners and communicate with residents, families, physicians, and staff.PHYSICAL REQUIREMENTS/WORKING CONDITIONS:This position requires the ability to perform the essential functions of the job with or without reasonable accommodation This position primarily uses a personal computer and general office equipment, including a printer, scanner, fax machine, copier, and telephone. The work environment may involve frequent interaction with residents, family members, staff, and others, including individuals who may be upset, vulnerable, or challenging to communicate with. The employee may be exposed to varying and unpredictable situations, emergency or crisis conditions, extreme temperatures, infectious or contagious diseases, and blood, bodily fluids, or tissue. Required personal protective equipment must be used as dictated by working conditions, policy, and applicable safety requirements.
