Compassionate Hospice RN Case Manager

Gentiva

Gentiva is seeking a Hospice RN / RN Case Manager to lead patients through their hospice journey, delivering skilled nursing care in patients’ homes or other settings. You will perform assessments, update care plans, and collaborate with an interdisciplinary team to ensure compassionate, compliant care for patients and families. Requirements include an active Texas RN license and at least 1 year of RN experience, with hospice or palliative care experience preferred. #J-18808-Ljbffr
