Established in 2003 in Muncie, First Choice Physical Therapy opened its second location in New Castle in 2006. We believe in placing the well-being of our patients first and we accomplish this through carefully selecting compassionate, dedicated, and team-oriented staff. Our personalized approach ensures patient comprehension and will enhance proper treatment progression to secure the best outcome for our patients. First Choice Physical Therapy takes pride in the level of personal and compassionate care that is delivered, emphasizing a 'hands on' approach to rehabilitation and exercise. Our staff is not only great at what we do every day, but we also love our profession, and we enjoy working with our community.
Job Description
We're looking for a driven Physical Therapist who's eager to grow! This role includes the potential to advance into a leadership position as you develop within our team. If you are a friendly and compassionate team player looking to grow indefinitely, you've found your forever career!
- Assess patients to develop and initiate treatment plans based on the evaluation results
- Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
- Provide direct patient care by established protocols and patient care guidelines
- Document findings, progress, and instructions to patients and caregivers
- Responsible for patient care performed by assistants and rehabilitation technicians
- Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
- Implement physical therapy treatment program and provide essential physical therapy treatments
- Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
- Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports
Qualifications
- Graduate degree from CAPTE-accredited Physical Therapy program. New graduates are encouraged to apply, we'd love for you to start your career with us!
- Current state of STATE license and CPR certification
- Ability to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans or willingness to learn
- Ability to work efficiently/effectively in an autonomous environment
- Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office,
- Exercise, sports rehab, and manual-based skills
