Job Summary:
Constant Care Health Services LLC is seeking a dedicated and compassionate Speech Therapist to join our team of healthcare professionals in The Bronx, New York. Responsibilities:
Assess and treat patients with communication and swallowing disorders in a home care setting
Develop and implement individualized treatment plans to achieve patient goals and improve quality of life
Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
Conduct frequent assessments and progress monitoring to adjust treatment plans as needed
Maintain accurate and detailed records of patient treatments and outcomes
Participate in multidisciplinary team meetings to share knowledge and best practices
Stay up-to-date with industry developments and advancements in speech therapy techniques and technology

Requirements:
Speech Therapist degree and licensure in the state of New York
1-2 years of experience (entry-level candidates encouraged to apply)
Ability to work independently in a home care setting
Strong communication and interpersonal skills
Ability to maintain accurate and detailed records
Familiarity with industry-standard software and technology
