Summary: Delivers primary health care services to patients and provides individual and/or group health education and counseling while providing outreach to and advocacy for the homeless population.

Essential Duties:

1. Delivers primary health care services to patients in the clinic and in the mobile healthcare unit.
2. Provides COVID services as needed in the clinic and in the mobile healthcare unit.
3. Provides individual and/or group health education and counseling while providing outreach to and advocacy for the homeless population.
4. Conducts physical examinations of patients. Exercises sound nursing judgment when initiating and prescribing therapy within the guidelines of written protocols.
5. Interviews patients, documents their medical history and makes assessments and/or physical diagnosis of condition.
6. Maintains proper clinical records and reports, using problem-oriented medical records which can be audited for accountability.
7. Participates in program Quality Assurance (QA) committee and quality improvement efforts.
8. Performs and completes PCMH duties as assigned. Duties include but are not limited to: attending patient huddle, timely clinical advice, involved in QI activities, and access needs.
9. Initiates medications and treatment within the scope of practice.
10. Supervises assigned clinical staff.
11. Refers patients for specialized medical care beyond the scope of the HCH Services, including arrangement for any necessary ambulance transportation.
12. Collaborates with the Clinical Director in review/revision of policies, protocols, and procedures as necessary.
13. Responsible for covering after-hour calls for FHWC.
14. Participates in and/or conducts in-service presentations.
15. Performs all other duties as assigned.

Qualifications

Education, Experience & Training: Must be licensed as a Registered Nurse Practitioner in the State of Kentucky; successful completion of a Nurse Practitioner training program in an accredited educational institution; and at least two years of broad-based professional nursing experience.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Position requires occasional or frequent moderate physical activity. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand; walk, climb, balance, stoop, kneel, crouch, crawl, and use hands to manipulate, handle or feel; and reach with hands and arms. The employee is occasionally required to lift/and or exert force up to 50 pounds.

Other Requirements: Must have a valid driver's license and the flexibility and willingness to change schedules and duties as needed according to staff availability. Must be willing to travel the service area as needed. Travel is primarily local or within the service area during the business day, although some out-of-area and overnight travel may be expected. Agency policy regarding drug and alcohol testing and criminal background check applies to this position. Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency.
