Home Health Registered Nurse

Job Title: Home Health Registered Nurse (RN) Location: Chicagoland, IL — Field-based Pay: $70,000 – $90,000 per year, based on experience Type: Permanent | Full-Time Job Summary: The Home Health RN provides skilled nursing care to patients in their homes across the Chicagoland area, managing a caseload of patients with acute and chronic conditions in coordination with physicians, therapists, and care coordinators. This role is ideal for clinicians who want autonomy, manageable caseloads, and the opportunity to deliver meaningful one‑on‑one care outside of a hospital or institutional setting. The position is part of a growing, Medicare and JCAHO‑certified home health operation serving communities across Illinois. Key Responsibilities: Conduct initial and ongoing patient assessments in the home setting, including OASIS documentation for Medicare patients Develop, implement, and update individualized care plans in collaboration with the interdisciplinary care team Administer skilled nursing interventions including wound care, IV therapy, medication management, and patient education Coordinate care transitions and communicate effectively with physicians, case managers, and family members Ensure timely and accurate clinical documentation in compliance with Medicare, Medicaid, and agency standards Monitor patient progress and adjust care plans based on clinical findings and physician orders Participate in care coordination meetings and contribute to quality improvement initiatives Must-Haves / Requirements: Active, unencumbered RN license in the state of Illinois 1+ years of clinical nursing experience, preferably in home health, med‑surg, or community health Valid driver’s license, reliable transportation, and current auto insurance Comfortable working independently in a field‑based role with minimal on‑site supervision Proficient in electronic health records and clinical documentation Preferred Qualifications: Prior home health experience with OASIS documentation Experience with Medicare and Medicaid patient populations Bilingual in English and Spanish or another language Screening Questions: Do you hold an active, unencumbered RN license in the state of Illinois?
