Develop and implement individualized treatment plans, including therapy for speech, language, voice, cognitive-communication deficits, and dysphagia management; monitor progress and adjust plans as needed. Provide direct therapy and supervise care plans in collaboration with an interdisciplinary team (nursing, OT, PT, dietitian, social work) to support functional communication and safe eating and swallowing. Perform appropriate documentation including evaluations, progress notes, treatment plans, and discharge summaries in accordance with regulatory and facility standards. Participate in care planning and discharge planning, providing education to residents, families, and staff to support ongoing communication strategies and safe swallow practices. Requirements Active Connecticut state license as a Speech-Language Pathologist (SLP) Master’s degree in Speech-Language Pathology from an accredited program ASHA Certification (CCC-SLP) preferred or eligible Clinical experience in speech-language pathology, with emphasis on acute care or long-term care settings preferred Ability to work in a long-term care environment and collaborate with an interdisciplinary team Eligibility to work in the United States Benefits and Support 35 guaranteed hours per week Dedicated travel and clinical support throughout the assignment Structured 13-week contract with potential for extension Access to credentialing and onboarding processes aligned with travel assignments Please note: Pay rates and guaranteed hours are estimates and may vary.
