Job Description:

Title: Physical Therapist

Requirements:

- Graduate of an accredited Physical Therapy program.

- Current Physical Therapist license in the state of practice.

- Experience in skilled nursing facility or geriatric rehabilitation preferred.

- Strong clinical, communication, and organizational skills.

- Ability to work both independently and as part of an interdisciplinary team.

Roles & Responsibilities:

- Evaluate residents' mobility, balance, strength, gait, and functional limitations through initial and ongoing assessments.

- Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability.

- Provide direct therapy interventions, including therapeutic exercises, gait training, and balance retraining.

- Monitor resident progress and adjust care plans to ensure achievement of therapy goals.

- Participate in interdisciplinary care team meetings and contribute to discharge planning.

- Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.

- Supervise and guide Physical Therapist Assistants and therapy aides as necessary.

- Accurately document treatment sessions, progress notes, and billing information in a timely manner.
