Physician Internal Medicine-Geriatrics - Competitive Salary

Job Description

• Primary Care Physician with focus on geriatric, community based, value based care

• Full-time employed position in an outpatient PACE model

• Average of approximately 8 patients per day with no hospital rounding

• Regular schedule Monday through Friday, 8 a.m. to 5 p.m., including some home visits

• Rotating phone only call a few nights or weekends per month

• Perform comprehensive in person assessments and lead the interdisciplinary team to create care plans

• Coordinate discharges and placement from acute, short term, and long term care settings

• Emphasis on interdisciplinary team based care for Medicare and Medi Cal eligible seniors

• Competitive base salary in the range of $275,246 to $330,595plus quarterly bonus eligibility

• MD or DO with completion of an ACGME accredited residency and doctorate level degree

• Active unrestricted state medical license, state controlled substance registration, and DEA required; CPR and First Aid required within 180 days of hire

• Board certification preferred in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics with 2 to 5 years of primary care experience in geriatrics preferred

• Located in North Hollywood, California near attractions including the NoHo Arts District, Universal Studios Hollywood, and Griffith Park

• Approximately 12 miles (about 25 minutes depending on traffic) to downtown Los Angeles and 25 miles (about 40 to 50 minutes) to Santa Monica and Pacific Coast beaches
