Registered Dietitian - Per Diem

Dietitian Position
Plans and delivers medical nutrition therapy to patients according to the physicians' diagnoses. Performs as a member of the multidisciplinary care team. Assesses all newly admitted patients to identify and address nutritional risk. Monitors and advises dietary employees and other support personnel. Ensures menus meet requirements of therapeutic diets based on guidelines established in the Nutrition Care Manual.
Education/ Experience:
- Bachelor's degree from an approved program by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
- Proof of passing the national examination administered by the Commission on Dietetic Registration (CDR).
- Participates in continuing educational requirements to maintain registration and certification in the state of Vermont
- Certified Dietitian, State of Vermont.
Other Requirements:
- Able to communicate effectively in English, both verbally and in writing.
- Additional languages preferred.
- Basic computer knowledge.
Essential Functions Of The Positions:
- Conducts patient nutrition interviews with patient and/or family and obtains necessary information from the medical record to determine patient's nutritional status.
- Provides medical nutrition therapy using the nutrition care process and follows standards as established by the Academy of Nutrition and Dietetics as outlined in the Online Nutrition Care Manual.
- Develops nutritional care plans for patients at high nutritional risk (of any age including geriatric and terminally ill patients).
- Evaluates the results of nutritional intervention on the patient's nutritional status to determine the need to revise the nutritional care plan.
- Interacts professionally with patient/family. Provides nutritional teaching and counseling to patient/family in making desired changes in eating habits.
- Annually reviews and complies with CMS guidelines and conditions of participation for CAH as it relates to clinical nutrition.
- Performs quality Improvement audits and reports that contribute to improving nutritional care to patients.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Attends and participates in weekly multidisciplinary patient care planning meetings or communicates via use of interdisciplinary team meeting document in medical records.
- Supervises dietary staff in conducting nutritional screening of patients.
- Monitors proper sanitation in the dietary kitchen by completing monthly sanitation checks.
- Assists in orienting and in-servicing new departmental employees
- Conducts nutrition analysis of menus and assists with menu development and revision.
- Assists in creating and delivering nutrition-specific departmental in-service.
- Maintains a good working relationship both within the department and with other departments.
- Documentation meets current standards and policies.
- Conducts training and counseling as requested to hospital staff and the community.
- Treats patients and their families with respect and dignity.
- Demonstrates an ability to be flexible, organized and function under stressful situations.
