About the Position

RN Inpatient Care Coordinator

Location: Sitka, AK
Pay Range: $41.66 - $58.69/hour
Sign-On Bonus: Up to $25,000
Relocation Assistance: Up to $10,000 for qualified candidates

Position Summary

We are seeking an experienced RN Inpatient Care Coordinator to support patients throughout their inpatient stay by coordinating care plans, facilitating discharge planning, collaborating with interdisciplinary teams, and ensuring continuity of care. This role combines strong clinical nursing skills with care coordination and patient advocacy to achieve optimal outcomes and a positive patient experience.

Responsibilities

• Conduct comprehensive patient assessments and identify care coordination needs
• Collaborate with physicians, nurses, therapists, social workers, and community resources
• Develop and manage individualized care and discharge plans
• Coordinate referrals, follow-up appointments, and post-discharge services
• Educate patients and families regarding treatment plans and available resources
• Monitor patient progress and address barriers to care
• Advocate for patients and assist with navigating the healthcare system
• Maintain accurate documentation within the electronic medical record
• Participate in quality improvement and patient safety initiatives

Qualifications

• Associate Degree in Nursing (BSN preferred)
• Active and unrestricted Registered Nurse license
• Current BLS certification required
• Strong clinical assessment, communication, and care coordination skills
• Acute care, case management, utilization review, discharge planning, or care coordination experience preferred
• Ability to work collaboratively within a multidisciplinary team
