Occupational Therapist (OT)

Position Summary

The Occupational Therapist (OT) is responsible for evaluating, developing, and implementing individualized occupational therapy treatment plans for residents in a skilled nursing facility. The OT works collaboratively with physicians, nursing staff, rehabilitation team members, residents, and families to improve functional independence, maximize safety, and enhance quality of life. Services are provided in accordance with physician orders, professional standards, Medicare guidelines, and all applicable federal and Florida state regulations.

Essential Job Functions

• Evaluate residents' physical, cognitive, sensory, and psychosocial functioning.

• Develop individualized treatment plans based on physician orders, clinical findings, and resident goals.

• Provide evidence-based occupational therapy interventions focused on:

  • Activities of Daily Living (ADLs)

  • Instrumental Activities of Daily Living (IADLs)

  • Upper extremity strength and range of motion

  • Fine motor coordination

  • Balance and functional mobility

  • Cognitive and perceptual retraining

  • Visual and sensory integration

  • Adaptive equipment training

  • Wheelchair positioning and seating

  • Home safety and discharge planning

• Perform standardized assessments and reassess residents as required.

• Modify treatment plans based on resident progress and changing medical status.

• Educate residents, families, and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.

• Recommend durable medical equipment (DME) and environmental modifications.

• Collaborate with Physical Therapists, Speech-Language Pathologists, nursing staff, physicians, and interdisciplinary team members.

• Participate in care plan meetings, discharge planning conferences, and family meetings.

• Maintain timely, accurate, and complete documentation in accordance with Medicare Part A, Part B, Medicaid, insurance, and facility policies.

• Complete evaluations, progress notes, recertifications, discharge summaries, and other required documentation.

• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with Florida law and facility policy.

• Maintain productivity expectations while ensuring quality resident care.

• Follow infection prevention, OSHA, HIPAA, and facility safety protocols.

• Participate in quality improvement initiatives and ongoing professional development.

QualificationsEducation

• Master's or Doctorate Degree in Occupational Therapy from an accredited program.

Licensure

• Current, active Florida Occupational Therapist license in good standing.

• Current CPR/BLS certification (or obtained within facility-required timeframe).

Experience

• Skilled Nursing Facility (SNF), rehabilitation, long-term care, or geriatric experience preferred.

• New graduates may be considered.

Knowledge, Skills & Abilities

• Strong knowledge of geriatric rehabilitation principles.

• Knowledge of Medicare Part A, Part B, PDPM, Medicaid, and rehabilitation documentation requirements.

• Ability to evaluate functional performance and develop individualized treatment plans.

• Knowledge of adaptive equipment and assistive technology.

• Excellent communication and interpersonal skills.

• Strong documentation and organizational abilities.

• Ability to work independently and collaboratively within an interdisciplinary team.

• Compassionate, patient-centered approach to resident care.

• Proficiency with electronic medical records (EMR).

Physical Requirements

• Frequently stand, walk, bend, reach, stoop, kneel, and lift.

• Ability to transfer and assist residents using safe body mechanics.

• Lift, push, or pull up to 50 pounds with assistance when necessary.

• Ability to perform repetitive hand movements and fine motor tasks.

• Visual and auditory acuity sufficient to evaluate and treat residents safely.

Working Conditions

• Skilled nursing and rehabilitation environment.

• Frequent interaction with medically complex geriatric residents.

• Exposure to infectious diseases and bodily fluids; appropriate PPE is provided.

• May require occasional weekend or holiday coverage based on facility needs.

Regulatory Requirements

• Maintain unrestricted Florida Occupational Therapist license.

• Comply with all federal, state, and local regulations, including:

  • Florida Practice Act

  • CMS regulations

  • Medicare/Medicaid guidelines

  • HIPAA Privacy Rule

  • OSHA standards

  • Facility policies and procedures

• Successfully complete all required background screening, drug testing, and mandatory in-service education.
