Develop and implement individualized treatment plans with measurable goals; provide therapy focused on communication, cognition, and appropriate swallowing interventions. Monitor resident progress, document outcomes, and update treatment plans as needed. Collaborate with an interdisciplinary team (nursing, therapy, social work) to coordinate care and optimize resident outcomes. Educate residents, families, and care partners on communication strategies and support needs. Ensure documentation and practice comply with regulatory and facility policies, including privacy and HIPAA guidelines. Requirements Arizona license required for submission or must have already applied and be able to provide ETA for license approval. Ability to obtain or maintain Arizona licensure during the assignment; ETA accepted for license approval. Availability for 32 hours per week with day shifts; 8-hour shifts; 4 days per week. Ability to work in a long-term care setting and adapt to a hospital-adjacent care environment as needed for resident needs.
