You'll manage a patient caseload while collaborating with physicians, social workers, hospice aides, spiritual care, bereavement counselors, volunteers, and other members of the hospice team to ensure every patient receives compassionate, individualized care. Patients are cared for wherever they call home, including: private residences, assisted living facilities, skilled nursing facilities, residential hospice, and other community settings.RequirementsDevelop and oversee individualized plans of careAssess patients and proactively manage symptoms and painEducate and support patients and families through every stage of the hospice journeyCoordinate medications, durable medical equipment, and specialty servicesCollaborate closely with physicians and the interdisciplinary teamParticipate in Morning Report and Interdisciplinary Group (IDG) meetingsProvide compassionate crisis intervention and symptom management when neededAdvocate for patients while ensuring exceptional quality of careQualificationsCurrent New York State Registered Nurse (RN) licenseMinimum of 2 years' experience as a Registered Professional NursePreferred strong clinical skills in bedside care, symptom management, crisis intervention, and family interventionAbility to work independently while collaborating with a multidisciplinary teamExcellent communication and organizational skillsSalary Description $41.21- $44.97
