Pay: Starting at $22.00 per hour Location: Phoenix Join Our Team Spectrum Healthcare is seeking a compassionate and dedicated Case Manager to support individuals with behavioral health needs by coordinating care, promoting wellness, and helping clients successfully navigate healthcare and community support systems. As a Case Manager, you will play a vital role in improving client outcomes through care coordination, treatment planning, supportive counseling, and collaboration with healthcare providers and community partners. What You'll Do Coordinate care and treatment activities for assigned clients. Participate in client staffing and implement individualized treatment plans. Provide supportive counseling and document services using the DIAP format. Complete assessments and reassessments under professional supervision. Develop and update treatment plans in accordance with documentation standards. Coordinate services with physicians, healthcare providers, community agencies, and support systems. Assist clients in navigating behavioral health and physical healthcare services. Maintain accurate, timely, and compliant client records. Participate in utilization review and performance improvement activities. Attend team meetings, trainings, and professional development opportunities. Maintain clinical productivity standards and quality care expectations. Promote culturally competent, client-centered care. High Needs Case Manager Responsibilities Provide services to individuals identified as High Needs, including those designated as Seriously Mentally Ill (SMI). Conduct regular community and home visits. Assist clients during hospital admissions, discharges, and transitions of care. Coordinate Child and Family Team (CFT) and Adult Recovery Team (ART) meetings. Collaborate with DCS, DDD, probation/parole, schools, healthcare providers, and community resources. Complete referrals, prior authorizations, and required health plan documentation. Qualifications Candidates must qualify as a Behavioral Health Technician (BHT) and meet one of the following: High school diploma or GED and at least 2 years of behavioral health experience; OR Bachelor's degree in any field and at least 1 year of behavioral health experience; OR Bachelor's degree in a human services-related field; OR Completion of a Behavioral Health Technician training program; OR Master's degree in a human services-related field and not independently licensed. Preferred Qualifications One year of behavioral health experience. Experience working with individuals with Serious Mental Illness (SMI). Case management or care coordination experience. Knowledge of community resources and healthcare systems. Skills & Knowledge Understanding of behavioral health treatment and recovery principles. Knowledge of DSM-5 diagnostic criteria. Ability to coordinate physical and behavioral healthcare services. Strong documentation and organizational skills. Crisis intervention and problem-solving abilities. Effective communication and relationship-building skills. Ability to work independently and collaboratively within a multidisciplinary team. Commitment to culturally responsive and trauma-informed care. Requirements Valid Arizona Driver's License. Ability to travel throughout the community as needed.
