St. Vincent’s Medical Center seeks a Parish Registered Nurse in Community Health to provide faith-informed, holistic nursing care in parish and community settings across Bridgeport and Fairfield County. The RN will conduct health assessments, manage chronic disease education, coordinate screenings and immunization clinics, and provide patient advocacy, referrals, and health coaching. Working closely with clergy, community partners, and interdisciplinary teams, this role promotes wellness, health equity, and preventive care while supporting care transitions from hospital to home in alignment with our mission and quality standards.
