Social Worker

Outstanding Service: Provide compassionate and effective support to residents while helping improve their quality of life. Continuous Improvement: Participate in meetings, training, and quality improvement initiatives to improve services and efficiency. Team Collaboration: Work collaboratively with healthcare professionals and staff while sharing knowledge and developing professional skills. Job Duties: Plan, organize, and supervise social services programs and staff. Ensure timely completion of resident assessments and required documentation. Ensure compliance with PASRR and applicable federal and state regulations. Assign, review, and evaluate the work of Social Services staff. Provide coaching, performance feedback, and staff supervision. Assist with interviewing, selecting, and training staff. Coordinate safe and appropriate discharge planning with residents, families, and healthcare providers. Monitor required Medicare notices and documentation. Coordinate referrals and consultations with healthcare specialists and community resources. Advocate for resident rights and assist residents and families with social service needs. Maintain a personal caseload and provide case management services. Assist with department budgeting and monitor expenditures. Participate in interdisciplinary meetings, committees, and quality improvement activities. Knowledge of: Federal and state healthcare regulations and CMS standards Social work principles and social services programs Geriatric behavioral health and medical conditions Community resources and government assistance programs Long-term care practices and MDS requirements Skills in: Verbal and written communication Counseling and case management Resident assessments and care planning Problem solving and critical thinking Staff supervision and management Ability to: Build positive relationships with residents, families, staff, and healthcare professionals Maintain accurate electronic records and documentation Handle confidential information professionally Work independently while effectively managing multiple priorities Required Qualifications: Master’s degree in Social Work (MSW) REQUIRED Active Arizona social work license upon placement One year of supervisory experience in a healthcare setting Long-term care experience preferred Ability to obtain and maintain an Arizona DPS Fingerprint Clearance Card #J-18808-Ljbffr
