OT - Occupational Therapist

Days, 8 hours M-F. Times are flexible as long as consistent and communicated with the Director of Rehab. Job Description The Occupational Therapist is responsible for screening, evaluating, and developing care plans for patients with cognitive and/or ADL physical functioning disorders. The role requires effective communication and collaboration with patients, families, caregivers, and other service providers. Responsibilities Screen and evaluate patients with cognitive and/or ADL physical functioning disorders. Develop appropriate plans of care adhering to regulatory and clinical practice standards. Utilize standardized assessments and evidence-based practice to support clinical interventions. Provide rehabilitative, skilled, and medically necessary treatment interventions. Administer various rehabilitation procedures including ADL retraining, therapeutic activities, and exercises. Document all required information in the electronic documentation system and maintain patient records. Assess patient progress and adjust treatments based on evaluation outcomes. Provide consultation and counseling to patients, families, and caregivers. Participate in patient care conferences, utilization review meetings, and required trainings. Supervise occupational therapy assistants and rehabilitation aides or students. Required Experience / Certifications / Licensure 1+ years of Occupational Therapy experience (REQUIRED). Experience in Long Term Care (LTC) or Rehabilitation (REQUIRED). Active Occupational Therapy license (REQUIRED). New graduates considered with Level II experience, not just Level I observation.
