Care Manager - Health Homes - Openings in various Counties

Position Overview Care Managers provide outreach and enrollment services to children presumed to meet eligibility requirements of New York State's Children's Health Home program. The Care Manager will gather assessments and enter documentation that confirms eligibility and identifies areas requiring supportive services. They work with children, families and service providers to create a comprehensive Plan of Health Care, identify additional service providers as necessary, and coordinate the continuing involvement of families and service providers in the execution of the plan. Care Managers document progress and arrange for eventual transition between health homes as necessary, or from health homes to other levels of service following changes in eligibility. Required Education, Knowledge, and Skills Associate's Degree required; Bachelor's Degree preferred. 2 Years relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services. Case load would be 1 to 20 individuals. Experience coordinating and participating in team settings. Solid writing and verbal communication. Work effectively and knowledgeably across a broad spectrum of cultural, ethnic, and racial communities. Ability to work in an effective and focused manner when self-directed and beyond immediate oversight. Ability to deliver family-driven, youth guided services. Valid NYS Driver's License required with clean MVR. May be required to complete online Defensive Driving course. Abilities and Working Conditions Must be available to work evenings and/or weekends. Must be able to lift and carry up to 25 pounds with or without reasonable accommodation. Willingness to respond to the needs of a culturally diverse population. Ability to deal positivity with changing priorities in a fast paced environment. Display a positive, professional and proactive attitude to accomplish quality work that is in line with the agency's mission statement. Seek options, alternative viewpoints in decision making. Exhibits a professional, courteous demeanor with internal and external constituents. Compensation Pay: $20.00 - $21.00 based off of education and experience.
